Become a Patient

Hello, and thank you for your interest in working with me to optimize your health! Below are the steps to schedule the Foundational Assessment and begin your journey with my practice.
Step 1: Complete a Pre-Screening Application
Please visit the Appointment Request Page and select “Screening Application” to fill out a compatibility questionnaire, which helps me determine if we are a good fit. When you complete the application, you will create a patient account through CharmHealth EHR, a HIPAA-secure electronic medical record system where your information will be safely stored. Please note that the “Screening Application” will be tied to a 10 minute appointment slot to give me time to review the form. It is not a real patient-visit appointment.
If we are a good match, I will invite you to schedule your Foundational Assessment through the patient portal or the same link above.
Step 2: Optional Discovery Call
If you have questions before scheduling the Foundational Assessment, you are welcome to schedule a 15-minute “Discovery Call” once your screening application is approved. This call is $50, which will be fully refunded if you schedule and complete the Foundational Assessment. This fee ensures that inquiries are from patients seriously considering joining the practice.
Step 3: Appointment Booking and Payment
You can schedule your Foundational Assessment through this link or directly through the CharmHealth Patient Portal. I require full payment in order to book your Foundational Assessment. I see patients on Tuesdays and Thursdays, morning through early afternoon.
Step 4: Prepare for Your Foundational Assessment
Before your appointment, you will need to complete several forms:
- Telehealth Consent Form
- Office, HIPAA, and Informed Consent Form
- New Patient Questionnaire
You will also receive a separate link for a nutritional assessment, a powerful tool that provides insight into your dietary patterns. This assessment takes approximately 20 minutes and is optional, but highly recommended for more precise and personalized recommendations.
*Please note that upon logging in for your initial telehealth session, you will be required to upload a valid government-issued photo identification (e.g., driver’s license) for identity verification purposes.
